Mob of the Dead: Guide to Obtain the Hell's Redeemer (Upgraded Hell's Retriever) | Only 1 in Coop?

Reports are coming in that the Redeemer is able to be equipped by all players in Co-op, so perhaps it was a glitch or something else on our end that Axel wasn't able to obtain it. Also, reports are coming in that it may NOT be 30 kills required but running an entire round with only the Retriever. Also, people have reported not being able to get it that could be due to a variety of reasons, we haven't narrowed it down yet: they have surpassed Round 15, they have obtained the Golden Spork, or they have obtained all the Blue Skulls. Any one of these MAY cause you to not be able to get the Redeemer but nothing can be confirmed for sure at this point. Furthermore, I highly doubt you can unlock the Redeemer when playing on "Easy" difficulty.
We knew there was something up with the lava pit near the first Beast spawn location near spawn on Mob of the Dead! The pit gives you 20 points per frag thrown in but looks like it also does much more: it upgrades the Hell's Retriever to the Hell's Redeemer! Sadly though, it does appear as though there can only be 1, even on co-op. :(
